After a cerebrovascular accident (CVA) also known as a stroke, the nervous system is damaged at the highest level in the brain. Any medical condition that interferes with the normal communication from the brain to the muscles can result in long term muscle spasm which, if severe and the muscles become shortened and scarred, is called a contracture. Long term muscle spasm is most commonly as a result of a medical condition. If this situation is not addressed then there are issues with balance and either the paraspinal muscles themselves and/or the gluteal muscles come into a permanent state of tension to try and correct the asymmetry. The muscles on the outside of the curve (convex) become stretched, thinner and weaker. The muscles on the inside (concave) part of the curve get shorter, tighter and stronger.
